At the end of the Handbook, there is an acknowledgement form. Please sign and return this form to Jennifer Richards (jennifer@resultstc.com) immediately as they are now PAST DUE. If you have already signed and turned in your form, THANK YOU!

There was a new Maintenance and Repair form rolled out a few weeks ago? Please make sure that your club is using the new form when submitting all maintenance requests. This form should have replaced the old form in your Ops & Logistics Folder.
